What is the Widal test for salmonellosis?

It is traditionally used for diagnosing typhoid and paratyphoid fevers, though its use may be limited due to various factors, including the potential for false-positive or false-negative results.

The Widal test works by detecting the agglutination (clumping) of Salmonella bacteria when exposed to a patient’s serum that contains the corresponding antibodies. If these antibodies are present, they bind to specific antigens on the bacteria, causing agglutination, which serves as a visual indicator of a positive reaction.

When is this test performed?

The Widal test is typically performed when there is a suspicion of an infection caused by bacteria from the Salmonella genus, particularly in cases where typhoid or paratyphoid fever is suspected. Here are some situations when this test is generally recommended:

• Symptoms indicative of typhoid fever: Patients presenting with symptoms such as high fever, headache, fatigue, swollen lymph nodes, rash, or abdominal pain may be advised to undergo this test.
• History of exposure: If a patient has a history of traveling to regions where typhoid or paratyphoid fever is prevalent or has been in contact with someone diagnosed with these diseases.
• Unexplained prolonged fever: When a patient experiences extended episodes of fever that cannot be explained by other causes, the Widal test may be useful for diagnosis.

• H antibodies are directed against the bacterial flagellar protein. These antibodies appear later in the infection and remain elevated for a longer period. An increase in H antibody titer may suggest a recent infection.
