What is sputum culture?

A sputum culture is a laboratory test designed to detect and identify bacteria or fungi that may cause infections in the lungs or respiratory tract. Sputum is a thick fluid that can be expelled from the lungs by coughing. To collect the sample, the patient is instructed to take a deep breath and cough to produce sputum, which is then sent to the laboratory for analysis.

In the lab, the sputum sample is placed on a specialized nutrient medium and incubated to promote the growth of potential microorganisms. After a certain period, the lab technician examines the sample under a microscope to identify microorganisms. If a pathogen is detected, further tests, such as an antibiotic susceptibility test (antibiogram), can be conducted to determine which antibiotics the pathogen is sensitive to, helping the doctor prescribe the most appropriate treatment.

When is the sputum (Bact/Mic) test done?

Sputum testing is essential for understanding the cause of respiratory symptoms and guiding appropriate treatment. It is recommended to perform a sputum analysis if you:

  • Cough up thick, discolored secretions, which may signify bacterial or fungal infection.
  • Experience difficulty breathing, chest pain, or a fever, indicating a possible inflammatory process or infection.

The first attacker on the respiratory tract is often a virus, which damages the mucous membranes and makes it easier for bacteria that are normally present in the nose or throat to invade. After a viral infection, bacterial complications can further worsen the condition.

Infection spreads through droplets—via speaking, coughing, or sneezing. Close physical contact, like kissing, can also transmit the infection.

Besides bacteria, fungi like Candida can cause lower respiratory tract inflammation, especially in individuals who have used antibiotics for an extended period. Given these potential causes, timely sputum testing is crucial for accurate diagnosis and treatment.

How is a sputum sample collected?

Sputum, or phlegm, is collected by expectoration. The patient is usually instructed to take a deep breath and then cough forcefully to expel sputum from the lower respiratory tract, taking care not to include saliva from the mouth. The sample is then collected in a sterile container. It is important for the sample to be as clean as possible to ensure accurate results. Before collecting the sample, the patient is often advised to rinse their mouth with plain water to reduce the risk of contamination from oral flora.

Preparation for sputum analysis (Bact/Mic)

Sputum can be collected at any time, but it’s important to gather the sample when it is thickest. Additionally, if the person is taking antibiotics, it is advisable to delay the analysis. Antibiotics can interfere with or stop the growth of harmful microorganisms, which may affect the accuracy of the test results.
