When is Inorganic Phosphorus in 24-Hour Urine Measured?

The test is indicated in cases of suspected osteoporosis/osteomalacia and the presence of bone changes such as metastases. Osteoporosis is a condition characterized by decreased bone density and an increased risk of fractures, while osteomalacia is a disorder of bone mineralization that occurs due to a deficiency of phosphorus and vitamin D. Therefore, this test can help assess the balance of phosphorus in the body and identify possible imbalances that may be associated with these conditions.

What Do Low Values Indicate?

Low levels of inorganic phosphorus can be associated with the following disorders:

  • Vitamin D deficiency
  • Primary and secondary hyperparathyroidism
  • Certain kidney diseases (Fanconi syndrome, renal tubular acidosis)
  • Use of glucocorticoids and parathyroid hormone, which can cause mobilization of inorganic phosphorus from the bones and increased secretion through the kidneys
  • Use of antacids, diuretics, and long-term steroid therapy, which can lead to significant reduction in the level of this mineral (hypophosphatemia)

What Do High Values Indicate?

Elevated levels of inorganic phosphorus in urine can be encountered in the following situations:

  • Hormonal imbalances (hypoparathyroidism or hyperthyroidism)
  • Osteoporosis or osteomalacia
  • Increased activity or stress on the bones

Reverse time: 1 day

Preparation: Prepare a container for collecting 24-hour urine (estimate whether one or more 2-liter bottles are needed). Discard the first morning urine sample (do not pour it into the bottle). Collect all other urine samples throughout the day and night into the bottle. Add the first morning urine sample of the following day to the bottle and deliver the entire quantity of urine to the laboratory for analysis. For certain analyses, it is necessary to add a preservative to the bottle first – ask the laboratory for instructions on this procedure.
