What is HBc total antibody?

HBc total antibody or total anti-HBc antibodies, has a central role in diagnosing and understanding hepatitis B virus (HBV) infection. These antibodies develop in response to the core of the hepatitis B virus. When we talk about total anti-HBc antibodies, we refer to the combination of IgM and IgG antibodies specific to this viral part.

The presence of HBc total antibody can indicate various stages of infection. For example, during an acute infection, both types of antibodies (IgM and IgG) may be present. Still, as the infection progresses to a chronic phase or resolves completely, IgM antibodies usually disappear, while IgG remains permanently, indicating previous exposure to the virus.

There is also a particularly significant time during the infection known as the “window period,” when the HBsAg (surface antigen) has already disappeared from the blood, but HBs antibodies have not yet been detected. In this period, the only detectable marker of infection is often precisely HBc total antibody.

When is this analysis performed?

The analysis for HBc total antibody is performed in several specific situations to provide deeper insight into the status and history of hepatitis B virus infection:

  • Differential diagnosis of hepatitis: If a person shows symptoms of hepatitis but has no detectable HBsAg, this analysis can help confirm that the infection is caused by the hepatitis B virus.
  • Window period: As mentioned, there is a period between the loss of detectable HBsAg and the appearance of anti-HBs antibodies when the only detectable marker of infection is HBc total At.
  • Determining chronic infection: If a person is positive for HBsAg for more than six months and positive for HBc total At, it may indicate a chronic infection with the hepatitis B virus.
  • Monitoring the outcome of the infection: In situations where a person may not have detectable HBsAg or anti-HBs, the presence of HBc total At can indicate a previous infection and potential immunity to HBV.
  • Testing potential donors: Before donating blood, organs, tissues, or sperm, potential donors can be tested to reduce the risk of transmitting the hepatitis B virus to recipients.
  • Testing pregnant women: Although pregnant women are routinely tested for HBsAg, testing for HBc total At can provide additional information about the potential risk of transmitting the virus to the newborn.

What is hepatitis B?

Hepatitis B is an infectious liver disease caused by the hepatitis B virus (HBV). This disease can range from a mild form to a serious, chronic condition that can lead to liver damage or death.

There are two main forms of hepatitis B:

  • Acute hepatitis B – a short-term infection that can lead to symptoms such as jaundice, dark urine, fatigue, abdominal pain, loss of appetite, and nausea. Many people, especially adults, who are infected with the hepatitis B virus will have an acute infection and then fully recover.
  • Chronic hepatitis B – a long-term infection that occurs when the virus remains in the body. This is more common in people who are infected as babies or young children. Chronic infection can lead to cirrhosis, liver cancer, or death.

Modes of transmission include sharing needles, unprotected sexual contact with an infected person, transmission from mother to child during childbirth, contact with the blood of an infected person, and through minor injuries to the skin or eyes.

Prevention

Prevention is essential when it comes to hepatitis B. The most successful prevention method is vaccination, which is advised for all newborns, individuals at higher risk of infection, and those living in regions where the infection is common.
